We would like to know how we can improve our service to you and how you perceive our surgery and staff.
We are in the process of setting up a face to face patient participation group of approx. 20 patients who will meet bi-annually initially and then quarterly. We will be seeking your views on various issues that will affect the surgery and the patients registered with us.
Up until this date, we have had a virtual group, but to enhance participation we have decided to make the group a face to face group. If this is of interest to you and you would like to be part of the second wave of this project, please read the information in the links below and return an application form to the surgery.
We aim to gather around 20 patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
If you would like to contact the PPG directly please email them on firstname.lastname@example.org
If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.
We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.
Many thanks for your assistance
