The service is designed to help individuals who are over 18 years old with a variety of non-medical challenges. Through signposting and support, the Connect for Health service aims to improve health and wellbeing and reduce social isolation. 

If you feeling anxious, lonely or low in mood or have practical issues you are struggling to deal with, our Social Prescribing Community Connectors are available to listen, signpost you to suitable services and offer support during this difficult time.
